Health Care Plans

As young people with Duchenne Muscular Dystrophy grow older, the need for a clear and accessible health care plan becomes essential. With the transition from paediatric to adult care, increasing numbers of professionals involved, and the introduction of new equipment and support documents like Transfer and Positioning Plans or Oral Eating and Drinking Care Plans, it can be difficult to keep everyone on the same page—especially across a team of several support people.

Recognising this need, we’ve worked in collaboration with hospital staff and nurses in the field to develop a comprehensive health care plan template. This plan brings all critical information into one accessible document, designed to support support workers, educators, medical professionals, and emergency staff in providing consistent, informed care.
