Assessment Packages
As an NDIS provider supporting young people with Duchenne Muscular Dystrophy (DMD) and other dystrophinopathies & neuromuscular conditions, we use assessments as the starting point for all services. We conduct assessments across various areas, including fatigue, adaptive behaviour, carer burden, and psychosocial behaviours, to identify both strengths and areas where capacity building is needed. These assessments are crucial in determining what is needed next to provide the best possible support for each individual. We offer two assessment packages, which can be chosen based on the individual's specific needs.

Standard Assessment Package
This package includes two ABAS-3 adaptive behaviour assessments
One interview with the parent/primary caregiver (1.5 hours)
One interview with either school, support, or work staff (1.5 hours)
The total package cost is $969.95, billed at $193.99 per hour for 5 hours in total (NDIS line item 15_613_0128_1_3). This covers the interviews, data entry, and 2 hours for generating a results and overview report, which can be shared with stakeholders. This package helps identify the individual’s strengths, pinpoints areas where capacity building is required, and informs any necessary strategies or interventions.

Comprehensive Assessment Package
This package includes everything from the Standard Package (two ABAS-3 adaptive behaviour assessments), plus any additional assessments that may be needed. These additional assessments can include, but are not limited to:
Zarit Burden Interview (Zarit Carer Burden Scale)
Paediatric Anxiety Rating Scale (PARS-3)
Paediatric Quality of Life Inventory Multidimensional Fatigue Scale (PedsQL Multidimensional Fatigue Scale)
Overt Behaviour Scale
The cost of this package includes the 5 hours for the standard assessments, plus 2 additional hours for extra assessments, totalling 7 hours. It is billed at $193.99 per hour (NDIS line item 15_613_0128_1_3), and additional assessments are decided in consultation with the team. This package provides a comprehensive overview of the individual’s skill set and can be used to complete Functional Capacity Assessments (FCAs) or shared with providers conducting FCAs. It is also useful for creating detailed reports for the NDIS, including evidence for plan reviews.
